This made me very happy! I like the way you portrayed Sam drifting in and out of that fugue state at the beginning. The way they tease each other, Dean getting Sam with a faceful of grave dirt, Sam popping Dean in the head with the cap. And, yes, Bobby would SO kick Dean's ass for not telling him Sam was out. J. Alfred Prufrock's credit card made me laugh.
Your version of Hell was rather terrifying, with the empty, silent road and the smeared stars. God providing a door, but allowing Sam to face his fear and walk out of Hell himself. I liked the little details of the fair - it sounds like something they really would enjoy.
